Spot the diagnosis -97

(1) A chain smoker has developed dry gangrene of his big toe of his right foot .
(2) A diabetic patient has developed wet gangrene of the toes.
(3) An old patient has developed gangrene of his toes . He is neither a smoker nor a diabetic .
(4) A young Lady has come with elevation of the skin on the upper chest which is butterfly shaped and pinkish.
